Document can be printed, and dropped off to City Hall with payment. 

If you wish to pay online please use this link and drop your forms off at City Hall. 

When paying online please use one of the following, Pool Pass, Swimming Lessons, Sports Payment. If you are signing up for multiple, please select Sports Payment and make sure the total is correct, if you live out of town add in the $5.00 out of town fee PER CHILD. You will be contacted for payment correction if needed.
